Have a change control log application where an admin/tech documents any changes that happen the the domain, security, etc. That is to be followed up with written description for our "book" that is essentially and inventory of the machine (RAM, processors, Winver, drive configurations, and where it plugs into the network). If it is an application server, the documentation is accompanied by a Visio document so that it can be visualized in a discussion.
