Yep, went to all those places, setting notifications, etc.  Even went to
the sounds list, and tried playing different sounds with no luck.  Also,
I did notice the two different volume sliders, and both have been set to
max for testing, with no luck.

Treo has two volumes: Phone volume & PDA volume.  It is contextual, so
make sure you're in "today" screen, then click volume up button.  You
should see a bubble with two volume bars:  make sure both are on
maximum.
 
Also, you said you enabled sound in sound settings:  just sounds tab, or
under notifications tab as well?  If not, check notifications tab,
select appropriate event from dropdown, and depending on the event type,
make sure that either checkbox is checked, or if no checkbox, ring type
is not set to none. E.g. If you select Event "Phone: Known Caller" then
make sure Ring Type is not None, and test Ring Tone by hitting Play
button.
