Hmm could the something up with the ISA not letting something thru right, Iphone sync basically works as long as the Iphone can see OWA. Do you have VPN? You can try having him VPN into work and redo the sync to the server bypassing the ISA, see if that's the issue. The odd part is that he gets his mailbox that means the connection works. You've tried domain/username already?
