We shopped around and found that <http://www.conferencecalls.com/> had the best rates, coverage, and features for our needs, which sound similar to yours. Set-up was trivial and quick -- you just need a credit card. One drawback is that they don't have any capability to serve multiple conferences on a single account. So if you give the number out to everyone, and two people schedule a call for the same time, they'll just end up on the same conference call without warning. Since accounts are pre-paid, having multiple accounts ties up money for each account. Not a lot, but it's annoying.
