Aside from what the others have mentioned, I've seen some hardware
problems (usually either malfunctioning hardware, or really badly out
of date video or NIC drivers) that bypassed a dump altogether, and
caused the machine to reboot from scratch.

No bluescreen, no nothing - just a reboot starting with the POST
sequence. Scary when it happens.

Fortunately, that's pretty rare.
