Actually for me it's 4 to temporarily enable scripting:
Click 1: on the NoScript icon
Click 2: "temp enable surveymonkey"
Click 3: off the NoScript menu
Click 4: reload page with scripting on -- I have "auto reload" disabled because 
I often have multiple tabs from the same site and I don't want to auto-reload 
all the pages if I need scripting on just one of the tabs.

And I manually clear "Temp Perms" after closing the site as otherwise they stay 
"on" until I close FF.

What can I say, I'm truly paranoid about this stuff ;-)
