This seems like something you could really easily build with the Issue Tracking list template. As an academic exercise it'd probably be worth your time to just play with it and get it going. Will probably take you a few hours. I've seen project managers in particular kludge together all manner of workflow based forms on top of SharePoint with that particular in-box template.
Depending on the version of SharePoint you have there are some actual workflow options.
