It depends on exactly what you want to achieve. As other have said, OpenDNS is free and does some basic filtering.
Moving up a bit, a Juniper (and I'm sure Cisco/Fortinet/Checkpoint etc.) firewall can log all traffic and has a very cheap option to add commercial grade URL filtering, but it will be a bit of an "all or nothing" and certainly the Juniper's seem to deal very much in IP addresses rather than being aware who a user is. I think much of it comes down to how much granularity/control you want.
