Do you a few calendars setup? The outlook sync is via Google exchange
server. I remember when I setup exchange from my iPhone I had to
select which calendars to sync with the iphone. So if I have two
calendars and HTC is configured to sync to calendar A but outlook is
configured to sync to calendar b you might see your scenario.

What happens when you create an entry in outlook and edit that entry
on the HTC do the changes sync back to Outlook? In other words not a
new entry but existing entry.

Google has a configuration site where you can select which calendars
to sync with Exchange.
