Hi I'm 99% sure that name of your plugin (NukeAdd.dylib) does not match the name in Iop::Description. Probably you forgot to change it from "Add" to "NukeAdd".

I have installed it in a separate Plugin folder and added that to the path in init.py and that works ok. When I update the plugin list Nuke can see the dylib file.
