I think you have to do it for the normal pass as well.
also, it is best to multiply the blue channel by -1 instead of inverting it 
using an invert node.

regarding fbx cameras, I always had problems with that, so I use the max2nuke 
script (you can find it on scriptspot.com)
to copy/paste cameras or axis from 3ds max to nuke (it has a little bug - if 
your framerange does not start at 0 you have to offset the keyframes manually 
in nuke).
