Renee and I think this input should look like:
checkbox - configure IPv6 (default checked)
checkbox - accept DHCP offered address (default checked)
checkbox - accept router offered prefix (default checked)
manually assigned addresses:
address / prefix
....
If the configure IPv6 box is unchecked, everything else should be
greyed out.
