Subject: Incident reporting


Dear All,

The ICB is currently considering a way forward with regards to a universal 
process of documenting and analysing significant incidents that occur in 
General Practice. One possible solution is to use a bespoke version of Datix 
platform which is widely used in Secondary Care.

As part of the business case, it is imperative that there is an understanding 
of the types of significant incidents that occur and the frequency of recording 
of these events.

Please can you complete the template attached for the quarter Jan 23-March 23 
populating the SI themes. The report will need to be submitted every 3 months. 
This should be ANONYMOUS however can you also include the list size of your 
practice.


If you have had any significant events/incidents during Q4 2023/24 please could 
you fill out and return to me. We need this back by COP 5th June.



This will be a quarterly ask going forward.
