Following today’s Clinical Leader’s Meeting, I think it would be helpful to continue discussions regarding outcomes from the Same Day Access Pilots and reservations some CDs had in the discussion which we can feed back to the ICB. This was also the theme of a debate at the LMC conference in November where GPs discussed the division of Same Day access from core services, which members were generally not in favour of. The key themes I picked up today were: * Ongoing funding * Reservations around mandating a hub model, including wrapping Access up into the Enhanced Service bundle (albeit with no additional funding attached for the spec) * Clinical triage models/ stratifying risk * Supervision of ARRS roles * Patient satisfaction * F/U for diagnostics ordered by hubs * Impact on continuity of care: https://www.kingsfund.org.uk/sites/default/files/field/field_document/continuity-care-patient-experience-gp-inquiry-research-paper-mar11.pdf * A ‘one size fits all model’ for practices who already have access models they are happy with or high levels of patient satisfaction with access * Impact on over/ underdiagnosis Another issue that was raised nationally is the potential impact on GPs when a higher proportion of workload is complex care management as low level, non-complex care is dealt with by other providers. This won’t have covered all the issues raised, but please attend the meeting where we can discuss further.
