I think it's meant to be text representing what's in the picture, so people who can't see the image can still get the meaning. So if the picture's of some kind of text then puth that text in the alt, or if it's a picture of Bob fishing make the alt text say "Bob fishing". Or just leave it blank if there's no useful or meaningful alternative.
