FYI, I've recently had to look at something similar. In this case I've used the Javascript engine in InDesign CS2 to report on all images and captions, writing a report to a file. Google for InDesign Scripting Guide and InDesign Scripting Reference. The DOM is extensive and I couldn't find many good examples, but the scripting seems like a very powerful way of programatically building content or extracting it. Let me know if you want more info.
