Description:
1) Change APIs that currently require DocumentStore implementations to live in
the same package.
2) Move useful convenience code MemoryDocumentStore into utility classes
(applyChanges, checkConditions & friends)
3) Refactor caching so it becomes generic, applies to all DocumentStore
implementations (allowing to remove caching-specific signatures from the API)
