Cycling shoes with laces are still available but I believe the newer velcro and/or 
rachet systems
are actually much better.

One of the problems with foot comfort while cycling is that the foot and shoe both 
change size
during the ride.  Laces can be tied initially so the pressure is distributed the way 
you want it.
Over time, however, the lace pressure will even out.

Velcro and rachet systems can be easily adjusted while riding.  The shoes I'm using 
currently are
the Sidi Energy with 3 racheting buckles per shoe.  These are the most comfortable 
shoes I've ever
had.  I use the ratcheting adjustment frequently on nearly every ride.  Would never go 
back to
lace-up shoes.

I should also mention that I wear a custom orthotic insole with metatarsal pad for 
additional
comfort.  I usually can't manage more than about 40km without the insole - the pain is 
just too
great.
