I've committed all patches against the ND 2.0 provider. The following changes,
either based on the patch submitted by Fab or otherwise missing, were not
included and require separate patches. If anyone is aware of other missing
items, please add them to the list. (And anyone feel free to knock these off
the list.)
EP:Bind - still not implemented
(I don't think Fab's patch was complete. I just need to know if it is.)
CQ/SRQ - group/affinity support is unused
(Requires changes throughout the stack)
no use of pre-compiled headers
(I just didn't get to this.)
CQ - still requires change to mapping of status values
(Need ndstatus.h updated in order to build.)
QP - need to verify that the correct pkey/port eventually get set
ND*:CreateInstance - can simplify error handling by not setting IND**
(I just didn't get to this.)
winverbs - move initguid
(I just didn't get to this.)
