Can you please specify the following information:

1.       Exact driver version and FW version, amount of HCAs installed on your 
servers

2.       Ways to repro (I saw it was random, but was it during some heavy I/O 
stress or disable/enable test etc.)

3.       If possible, provide the output of "!process 0 7" from windbg

4.       Provide a debug dump (minidump)

>From 3 and 4 one can see whether OFA/MLNX drivers  was running when BSOD  
>occured

Hello, we are experiencing a number of random BSOD crashes on W2008R2 servers 
that have HP/Mellanox 26428 cards. Have tried both Mellanox and latest WinOF 
drivers, but still same issue. Analysing crash dump inconclusive but we have 
exactly the same config on other servers without Mellanox cards and they are 
stable, so suspect a driver issue. Have logged a call with MS and awaiting 
their response, but any guidance appreciated.
