You'll have to break up the text in some way, but you have a lot of options.
You could put borders around the OGC, or use a different font, or separate
your material into a different section of the publication. Anything you can
think of that you can use to make the OGC "easily identifiable" will work.
Then you describe the methods you used in the back next to the license.
It is not necessary to indicate which OGC came from which publisher.

You need to include the Death in Freeport copyright notice, and the Wizards
copyright notice. However, since the Death in Freeport notice also includes
the Wizards notice, I don't see any need to put Wizards in twice. Wizards
comes first, then Green Ronin, then yours.
