I think the only requirement is to copy the text of Section 15 exactly
as it appears in the work that you derived from. I would say that the
documents on wizards.com would be definitive in this particular case.
Since the legal section of the SRD at www.wizards.com/d20 lists the
authors names, I believe that you need to include them just as they
appear in Section 15 of the OGL license for the SRD.

As an aside, many people on the list appear to agree that it is not
necessary to repeat the SRD copyright (or any other duplicate entry) if
you derive from two sources that both list the SRD (or other source) in
Section 15. That is, you only need to list the SRD copyright notice
once, even if you use two sources that both list the SRD in their
Section 15 notice.
