If you want to credit someone, simply shoot them an e-mail and ask.  If 
they say "yes," print out the reply, file it in your "legal stuff" 
folder, and do it.  (If they say "yes, but...", decide if you can follow 
the "but...".  Then, do so, file all releated messages in the 'legal 
stuff' folder, and give credit.)

There are a bunch of possible reasons for someone to refuse permission, 
or to give very specific terms.  It might improve your success rate if 
you use the OGL'd items exactly as written, rave about them, and offer 
to show the final text to the person, including the text of the credit 
line.  

Of course, take my advice with some grains of salt.  If I really knew 
how to run a d20 business, I'd probably be doing that right now.
