has been updated to track the mailing list discussion. There is a change
log at the end and "new" images on new or changed sections. Please review
and comment - we have resolved many issues but still need to work some
things through, and I'm sure glXGetProcAddressARB() is good for another
dozen go-arounds on context independence :-)
