It seems that the document is corrupted.

1. Okular does show the content. It can be reached near the end of the
scrolling area on the right border of the Okular window. The rendering of the
first page is broken.
2. QPDFView shows the first page as thin vertical line (the same poppler
version is used).
3. Foxit (under wine) does not show the first (phantom) page at all.
4. ToC is broken for all applications.
5. pdf-tools ( http://www.pdf-tools.com/pdf/validate-pdfa-online.aspx )
validator shows the following log for this file:

Validating file "ddd.pdf" for conformance level pdfa-1b
 The separator before an 'endobj' must be an EOL. (279)
 The key Metadata is required but missing.
 The separator before 'endstream' must be an EOL. (280)
