If you look at any of my recent author merges (
http://openlibrary.org/recentchanges/merge-authors.json), you should be
able to find plenty of other examples too.

For example, http://openlibrary.org/search/authors?q=edward+o+wilson shows
three search hits (each with different numbers of works) which, if you
click through, you can see have all been merged with
http://openlibrary.org/authors/OL141176A/Edward_Osborne_Wilson

When I put together the spreadsheet of author merge URLs, I noticed that
there were a number of author entries from the LC imports a year ago which
didn't show up in the search index at all. I was generating the merges
from the database dump, but if you did an online search by name, the
authors wouldn't show up.

Another problematic scenario is creating new author records when editing a
work. Because the authors don't get indexed, their impossible to
find/reuse again.
