If you only want to backup the Check_MK configuration (~/etc/check_mk), I'd say the only backing up the master will be sufficient, as this instance knows all the configuration for all sites.
If you also need service inventory (~/var/check_mk/autochecks) or the rrd files (~/var/pnp4nagios/perfdata), then you'd need the back-up every instance separately.
