ms-doc, containing an example

Open it with ms-word: the headings are numbered, and their numbers are also
displayed in the table of contents.
Open it with openoffice writer: only the first heading has a number, the rest
does not. And in the table of contents the same, and the formatting is also not
correct: between number and heading's text there is no space.
