To change the numbering style for top-level document headings--1, 2, 3 for chapters; A, B, C for appendices--it is necessary to use paragraph numbering instead of Tools > Outline Numbering. The paragraph numbering is picked up correctly as the top-level numbering for reference fields and for the top-level entries in the ToC. Sub-headings in the ToC, however, do not pick up the correct top-level numbering when it comes from paragraph numbering. This means it is not possible to have correct ToC pages when the document uses chapter-page numbering style.
