Having two files with names differing in capitalization only is really asking for trouble; ultimately this is managed by the filesystem and not by OpenOffice, but it's normal that OpenOffice gets confused there.

standard.dic is not the standard dictionary but the generic user dictionary (i.e., it is an initially empty file where you add the words that you use often but that shouldn't be in the English dictionary, like the name of the street you live in). There is only one standard.dic. You can create new user dictionary files (e.g., one for places, one for people, one for technical terms related to your job) in the dialog you mention before, and that is the recommended way to do it.
