This can happen in only case: there is no consistency between number of
free blocks in storage blocks map and number of free blocks in storage
"super block". There was a bug which can lead to this under heavy load, but
it was fixed long time ago, and I didn't see it since that time. what is your
request rate? Does you have some free space on disks?
