Is there any Oops-Documenatation that explains what the combination
TCP_REFRESH_MISS/200 <-> NONE/<address> means? does this mean
that the requested object is fetched from <address>? then Calamaris
should report that as a successful transaction. In Squid, NONE/*
always shows up as NONE/- which means that no outside-action happens.
And the other sort is: what does TCP_ERROR mean? it happens (in your
provided lines) together with a 2xx-code and a successful DIRECT-Action.
