Not sure I can help here for the update of this file.
rexxref.pdf : some entries in the index are "??"
This happens also in trunk. A few months ago, I spent some time to try to
fix that, without success (my mail sent on june 20).
Not part of the doc, but I don't see a CHANGES file for 4.1.0.
If no one is on the subject, then I can initialize the file from the
trackers (in this case, remind me the procedure...)
