I poked Ben about this on IRC (mainly because I suspected it wouldn't
work for us if we wanted to turn it on) and we determined it is likely
a missing or outdated action_trigger_filters.json in the conf directory.
By default A/T only touches checkout.due, the json file there adds
some hold request entries as well.

We copied the "Hold Expires from Shelf Soon" action trigger and assigned it
to the library accordingly.
It was noticed that our copy of the original action trigger was missing the
event_parameter of "check_email_notify" but after added still no email was
sent.
All of our other email notices are going out, so we aren't sure what we are
missing.
Any guidance would be greatly appreciated.
