I think this is a fine suggestion and I can see the use of it in a number of situations. This should be added as a wishlist item on LaunchPad.
I would make the following two suggestions about the feature: 1. Use of the photo should be system optional (not that a person can opt-out; the system should be able to opt-out entirely) 2. In institutions/consortia where they have opted not to use this feature, there should be no indication of it in a patron record (like "Photo not found" or a broken image spot or even a blank space to give the impression something ought to be present).
