CODING: add code review guidelines
Attempt to enumerate things that reviewers should look for, to ensure
a consistent quality of review. This should be a good start; we can
add to it in the future as we discover more things that we'd like
reviewers to consider.
As a side effect, attempt to enumerate the bits of C99 that we rely on,
past the C89 baseline that we otherwise assume.
