To follow up on my self, a patch can be found at http://www.pdc.kth.se/~joda/vcache-diff
I think it should be just about it, not including hidden references, but they will be fairly easy to find when you change the vcache definition. Stuff still casting to vcache include QTOV and CBQTOV, but I think these do the right thing. I haven't looked at kdump, which will need some massage too.
