On March 20, DELTA install-cfgafs-20050320 for

   src/export/Makefile.in

Unfortunately, there is a strange confusion with the version numbers of
that file, with

   1.17 Wed Aug 25 07:11:34 2004 UTC
   1.15.2.1 Wed Aug 25 07:11:34 2004 UTC
   1.15.2.2 Wed Aug 25 07:13:48 2004 UTC
   1.18 Sun Mar 20 20:26:19 2005 UTC

I'm not sure what you think the "confusion" is.
Your patch was applied to the trunk at 20-Mar-2005 20:26:19 UTC.
Your patch has not been pulled up to the openafs-stable-1_4_x branch, which is 1.15.1 in this file.

In any case, the mentioned patch didn't make it into 1.3.81 released to
day. How are patches from CVS propagated to releases?

By being pulled up to the release branch sometime prior to the release.
