I can't explain this until I know why the pioctl() function is failing. There are many places this can fail.

Clearly what needs to be done at this point is to debug the state of
the AFS SMB Server at logoff.  First, the sys/pioctl_nt.c file must be
edited to log errors to a file or to the Windows Application Event Log.

Once it can be determined why the pioctl() functions are failing we
will know if the state of the AFS SMB Server can be probed.  You want
to know whether or not the AFS SMB Server thinks it has tokens for the
user or not.  You also want to know if the path is considered accessible
and what the ACLs for the path are.  This information can be queried
by pioctl() operations but not if the pioctl() calls are failing.
