Not that it should be important in this case but why are you running with Freelance mode off and without the loopback adapter? Failure to run with the loopback adapter will cause serious problems if the ip address is dropped temporarily for any reason.
The reason I asked about McAfee is that we know that certain versions of McAfee have caused problems in the past by attempting to write file data to AFS without using the correct file modes. You are going to have to follow the directions in the release notes to configure OpenAFS to export debugging data and then capture it using SysInternals' File Monitor and Debug Viewer. This will provide me the necessary data to determine what is failing.
