When the callbacks are broken on the volume, the AFS client will notify
the Explorer Shell that something changed.  When the Explorer Shell
attempts to re-read it will be told NOSUCHPATH.

If Explorer crashed, its a bug in Windows.  Send the crash report to
them.  I'm sure the Shell team will be interested.
