We currently run with a cache set at boot time at 75% of the partition size, and this has reduced the frequency of the problem to close enough to zero for us. At previous higher values (85% ??) we still saw this on an infrequent but regular basis (across 100s of hosts).

Every one of our boxes have 150MB /cache partitions and
cachesizes set to no more than 80MB.

Here's what I've determined so far from untarring this
particular 3.5GB tar file.

FAIL : Linux 2.4 (ext3, x86)
GOOD : Linux 2.4 (ext3, x86) diff. host than above
FAIL : Linux 2.4 (ext3, x86_64)
GOOD : Linux 2.6 (ext3, x86_64)
GOOD : Solaris 9 (ufs, SPARC)
