We have sort of gone down the IIS road at this point.  The Apache
mod_x-sendfile option seems to be the correct functionality whereby the web
server layer and NOT the application layer is doing the heavy lifting of
sending the file.   I found a company that has an Apache emulator ISAPI that
is willing to add that functionality to their product:
