I can't take a look at it right now, but I am going to guess that the form (ajax or plain) isn't providing the CHUNKS argument. I used the jquery_ui_widget.html rather than the custom.html example. I know my regular form didn't have that. Hopefully that is what it is. Glad to have eliminated OBD as a possible cause.
