This forces the output timing to fulfill
   dTS = dSegNo * fixedPacketDuration
where dSegNo = seqNo - lastSeqNo.

If timestamp patching is enabled, the output timestamp will be set
to lastTimestamp + dTS. This kind of relative updating is used to
handle seqNo- and timestamp-wraparounds properly.
