If we have the chance to support with one structure both systems
(online-ca and offline-ca) then it is not a big disadvantage to have a
directory with only one subdirectory. This is only a problem if nobody
knows about the reason and PKIs are not established by total beginners.

I think it's enough if we write a README which is placed in the
directory where ca/ or ext/ is placed.
