Hi Bahaa,
I think first we have to analyze what you are really seeing (I'm a bit confused).

I don't think the OpenCA::Server->run is killing the server because even though after the openca_start script returns "Failed" and the kernel kills a process I can connect and authenticate to the server using a web browser. I basically can run some commands but not all. Do you have an idea on where should I dig next?
1. All debugging stuff which I see until now from you shows me a correct server startup. If the daemon starts then it is usual that the process which starts the daemon exits after the startup.
2. If the startup script returns failed then please check before and after the startup how many openca processes are running. Before the start there should be 0 processes and after the startup 2.
3. Which process is killed by the "kernel"?
4. Which commands fail? Can you activate the debugging and then execute the failing programs. Perhaps there is some useful output in var/log/stderr.log.
