So the situation is that you can approve a request but the signature is not

I need more info on the system used and software versions (PERL, OpenCA,
OpenCA::DB/OpenCA::DBI, etc...).
Also try this check-list:
o Did you initialized the dB module BEFORE doing anything else
on the CA ???
o Is the user the web server is running as allowed to access/write
the CA/dB dir and files ???
