Did you activate Javascript? Did you own a certificate? If you want to
approve a request you must sign the request and you need a certificate
to do this.

This looks like your cacert.der is not present on the RAServer (perhaps
not imported) or something with the script goes wrong (perhaps a
misconfigured path).
