The problem is that you don't have a valid certificate on the RA Operator's
Browser to sign the approved request with. Check your "user certificates"
and, if you forgot to do so, use on the CA the openca-newcert script to
generate the Operator's certificate and then use the openca-browserexport
one to generate the .p12 importable file.
